Description of Service
Life Trak is a wristband program to assist residents who have medical conditions that may cause them to wander. The project is a rapid response partnership between law enforcement and the families of those individuals with conditions such as Alzheimer’s, Dementia, Down Syndrome, Autism, Prader-Willi Syndrome, and various brain injuries.

Participants wear a personalized wristband that emits a tracking signal 24 hours a day. Not available for individuals residing in care facilities providing 24 hour/7 day a week care. Individuals attending school or day programs may participate in the program provided there is an understanding of the Life Trak program.
Individuals with medical conditions that might cause them to wander. Potential clients must have a 24/7 caregiver and reside within the program coverage area.
