Description of ServiceSupports families to manage a wide range of questions, concerns and services for their child with special health care needs through:

- Finding medical care, health and community services
- Identifying financial resources (including insurance assistance)
- Family support groups
- Transition services
- Individualized care coordination
- Access to specialty care through referrals to and from specialized care and hosting specialty clinics in rural locations
