Description of Service
Provides scheduled patient advocacy and community care coordination services. Provides community engagement services, community education, facilitation, fiscal sponsorship, and community-based care coordination services by strengthening community and clinical connections for care coordination and addressing gaps in the social factors of health outcomes.
Grand, Jackson, Moffatt, Routt, Rio Blanco counties

Call or email to access services. Individuals must be able to communicate needs and existing resources. Also, be willing to complete a comprehensive self-assessment of the following:
- Medical and social needs
- Identifying existing resources
- Goal setting
- Motivational and supportive services to reach individual goals
Does not provide crisis services nor transportation services. Staff meet people in the community or home when appropriate.
