Intake Procedure
Call to set up an appointment to complete an intake. The following is needed to complete the intake, which takes 3 hours to complete:
- Initial interview
- Medical exam
- Medical and social history
- Physical examination
- Blood and urine test
- Tuberculosis test

Must be:
- 18 years of age or older
- Able to consent in writing to voluntary participation in treatment
- Dependent on opiate drugs for a one year period prior to admission to
