Description of Service
Provides the following long-term care programs:
- Adult Foster Care
- Alternative Care Facilities
- Home Care Allowance
- Home and Community Based Services
- Nursing Home Institutionalization
Clients may be eligible for these services depending on their income, age, if they have any disabilities, and what types of assistance they are already receiving (i.e. Health First Colorado, Supplemental Security Income, Old Age Pension).
Walk in to the DHS offices located at 1200 Federal Blvd, or the Montbello Satellite Office at 4685 Peoria St (for residents of zip codes 80238, 80239, or 80249) to request an application. Applications may also be submitted online through Colorado PEAK at https://coloradopeak.secure.force.com, or by mail or fax.
Proof of Residency
Residents of the City and County of Denver who are US citizens, naturalized citizens, or legal aliens
