General Information
Description of ServicePACE (Program of All-inclusive Care for the Elderly) is a Medicare program to assist seniors ages 55 and older with disabilities in living as independently as possible in their own homes and communities. PACE provides coordinated personal and skilled care in the home, as well as Day Health Centers where socialization, meals, and recreation are available. Additional services include:
- Home modifications
- Computer access
- Prescriptions delivered to the clients home
- Transportation to all medical appointments

- Proof of citizenship or permanent legal resident status of at least 5 years
