| Description of Program | Early Childhood Connections in conjunction with the ECHO Screening & Evaluation Program offers in-depth developmental evaluations using certified therapists and early childhood special educators for children less than 3 years of age if a problem in development is suspected. A Service Coordinator helps the family assess their resources and identify needed services such as therapy or Family Support Services. The Individual Family Services Plan identifies the child's needs and the family's priorities for their child. The plan is reviewed every 6 months. Together the family and Service Provider develop a Transition Plan to prepare for a change from infant/toddler services to preschool special education services. |
