General Information
Description of Service
Serves children from birth to age 21 that have, or are at increased risk for having, a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services of a type or amount beyond that required by children and youth generally. The HCP can help with the following:
- Medical Care/Medical Home
- Specialty Care
- Funding Resources for health care
- Community Resources
- Support Groups

Intake Procedure
Call to inquire about or to refer a child for HCP services. Information can be taken over the phone or an electronic referral can be sent to any interested person/community partner or agency to complete and return.

Eligibility
Children and youth with special health care needs from birth to 21 years. There are no diagnosis or income restrictions. All families, regardless of gender identity or language spoken, will be eligible for services. Also able to serve documented, undocumented, uninsured and insured children living in Weld County.

Payment Notes
