Description of Service
The Food Assistance program, also known as Supplemental Nutrition Assistance Program (SNAP), offers nutrition assistance to eligible, low-income individuals and families. Jefferson County works with State agencies, nutrition educators, and neighborhood and faith-based organizations to ensure that those eligible for nutrition assistance can make informed decisions about applying for the program and can access benefits.
Beginning October 1st, 2018, dial 2-1-1 to start a SNAP application over the telephone.
Complete an application online using PEAK. Call or walk in to receive an application by mail or in person. Once an application is received, an interview appointment is scheduled to determine eligibility. The interview can be completed over the phone or in person.
Documentation that may be required:
- Identification for the head of household
- Social Security number for all household members
- Proof of monthly income
If not available, sign and submit an application and the Department will follow-up.
Colorado PEAK provides a questionnaire that will give people answers about whether or not they may qualify for benefits for programs that provide food assistance, cash assistance, WIC supplemental nutrition benefits, and other types of assistance. The MyCOBenefits app is available to individuals who receive SNAP, Colorado Works, and Adult Financial benefits and is available in both the Apple and Android app stores. The app allows users to access their benefit information, report changes, check EBT card balances and more.
