Description of Service
Offers the CDC's Diabetes Prevention Program via distance learning, which is a year-long program developed for people who have been diagnosed with pre-diabetes. To reduce the risk of developing Type 2 diabetes, participants work to:
- Lose weight
- Engage in regular physical activity
- Adopt a healthier lifestyle
The program is provided online by a bilingual team of professionals and paraprofessionals.

Intake Procedure
Call for enrollment during regular office hours.
Intake Requirements
Individuals will be asked to provide:
- Address
- Phone number
- Referring MD name and location
- Starting weight and height

Classes are offered outside of regular business hours for convenience.

Individuals who have one of the following test results and can provide date of bloodwork and referring physician:
- Elevated A1C
- Elevated CDC Risk Test
- Elevated ADA Risk Test
