Description of ServiceOffers the CDC's Diabetes Prevention Program via distance learning, which is a year-long program developed for people who have been diagnosed with pre-diabetes. To reduce the risk of developing Type 2 diabetes, participants work to:
- Lose weight
- Engage in regular physical activity
- Adopt a healthier lifestyle

The program is provided online by a bilingual team of professionals and paraprofessionals.

- Referring MD name and location
- Starting weight and height
