Intake RequirementsApplicants will need to provide the following:
- Application
- Audiogram (no older than 2 years)
- Letter of reference from nonfamily member
- HIPPA form
- Physician's assessment
Helpful TipsService dogs have public access rights to accompany their deaf/hard of hearing teammates everywhere, including stores, restaurants, and on public transportation.
